The FDA BSE/Ruminant Feed Inspections Firms Inventory Data report displays a list of all active firms that have been inspected under the BSE/Ruminant Feed Inspections program. Data for the report is generated based on inspections tracked and completed in the FDA Office of Regulatory Affairs system, Field Accomplishments and Compliance Tracking System (FACTS) database.

District decisions listed in this report reflect the compliance status of firms when the report was generated. These district decisions may or may not represent the final Agency determination of compliance for these firms.

NOTE that BSE has not been identified within the United States.

This web page will allow users to perform a search by entering filter criteria in order to view the firms. Users can search the firms by Firm Establishment Identifier (FEI) or by performing an Advanced Search on selective filter criteria such as: Firm Name, District, State, Program Risk Identifier, BSE Firm Type, Last BSE Inspection Dates, etc.

In addition, a complete list of firms is available as an Excel spreadsheet.

The Data reported as of indicates the date when the firm data was last exported from the FACTS application.

The entire list of firms is available to be viewed through the browser by clicking on the Submit button or downloaded to an Excel spreadsheet by clicking on the Display Entire FDA BSE/Ruminant Feed Inspections Firm Inventory Report as an Excel Spreadsheet link.

Search for a firm using the Firm ID (FEI):

1. Select the Firm ID (FEI) radio button
2. Enter the FEI number in the field
3. Click on the Submit button
4. The FDA BSE/Ruminant Feed Inspections Firms Inventory Report opens and the information related to the FEI queried is displayed
5. Click on the Perform new search link to return to the search screen

Search for firms using the Advanced Search option:

The Advanced Search option allows users to enter specific information to narrow down their search using multiple search criteria.

1. Select the Advanced Search radio button
2. Enter as many or as few criteria in the fields as desired
3. Click on the Submit button
4. The FDA BSE/Ruminant Feed Inspections Firms Inventory Report is displayed with all firms matching the criteria entered
5. Click on the Perform new search link to return to the search screen.

Note: Users may also generate the entire FDA BSE/Ruminant Feed Inspections Firms Inventory by clicking on the Submit button without specifying any search criteria.

Validation rules:

• List boxes: a maximum of 5 options can be selected in each of the list boxes (FDA District, State, Firm Type(s), Last BSE District Decision) by holding down the Ctrl key.
  
• Firm Name: users may enter partial words contained in the name of the firm. The firm name field is not case sensitive, i.e. the search will return the same results if the user enters “Anderson” or “anderson”.
  
• Last BSE Inspection Date: enter a date range in the From and To fields in the format of mm/dd/yyyy. The From date must be earlier than the To date. To search on a specific date, enter the same date in the From and To fields.
  
• Sort By: a maximum of 3 options can be selected to sort the Web report by holding down the ctrl key. The hierarchy of the sort is based on the order in which the fields appear in the ‘Sort By’ list box. As an example, if State and FDA District are selected, the data will first be sorted by FDA District and then by the State since that is the order in which they appear on the list. As an alternative, users can search on the field that has a higher priority and then sort on the field that has a lower priority, i.e., search by FDA District and sort by State. If no sort criteria is selected, the results will be sorted by FDA District, State, and Firm Name.
