The searchable BMIS database includes entities (i.e., CIs, CROs, or IRBs) referenced in a Form FDA 1572 or 1571 received by FDA since October 1, 2008. The data are updated quarterly. Requests for other OSI records may be submitted to the FDA under the Freedom of Information Act (FOIA). Search results display up to 500 results based on record creation.
