FDA FOIA On-Line Request Submission Information

list item marker A complete description of your request must be in the Subject field.
list item marker An electronic copy of your Request letter may be attached to the Request form.
list item marker Multiple request may be entered without re-entering the Requester information.
list item marker After submitting a request you will be given a confirmation # and the ability to print copy of your request.
list item marker After acceptance by FDA FOI staff an electronic Acknowledgement letter with your FOI Control # will be sent to the e-mail address provided in the Request
