Event Description
It was reported that the offset stem adapter (p/n: 151304000) was not able to use because the surgeon could not twist it by using the dedicated wrench during "revision" surgery.The surgery was completed without problems by using another same offset stem adapter.There was a 30 minutes surgical delay and there was no information about the cause of revise.
