Device has been returned for evaluation.The customer¿s complaint of front panel switches not functioning was confirmed.The service technician observed front panel was damaged.The service technician ensured that the unit was fully functional after the repair.The cause was traced to component failure.No further information was reported.
 
Event Description
The customer reported to olympus during an endoscopic retrograde cholangiopancreatography procedure, the front panel lights remained on and the buttons were not functioning.The image was displayed much smaller, as if it was in picture in picture module instead of full screen.The procedure was completed with the same device.There was no patient injury reported.

This supplemental report is submitted to provide the results of the legal manufacturer¿s investigation and device history record (dhr) review.Updates to sections h4 and h6.The dhr was reviewed for the subject device.No anomalies were noted and it was verified the device was manufactured in accordance with documented specifications and procedures.The legal manufacturer performed an investigation and was unable to determine a conclusive root cause for the component failure.
