Description of event according to short form complaint filed: it is alleged that "[pt] received a cook celect filter on (b)(6) 2013.It is alleged that the [pt] was injured without further explanation.Hospital and medical records have been requested but not yet provided.".
 
Event Description
Patient allegedly received an implant via the right jugular vein due to pulmonary embolism.The plaintiff alleges tilt and vena cava perforation.The patient further alleges anxiety.Per a report from computed tomography; ¿positive for caval perforation.Superior extent of ivc filter mild l2 vertebral body.Inferior extent inferior l3 vertebral body.A total of 4 prongs have perforated the ivc.Maximum distance prongs perforated 8mm.Coronal images 0 degree tilt.Sagittal images 4 degree tilt anterior posterior.Diameter of ivc directly above filter 20mm x 12mm.¿.

Per a report from computed tomography 2; ¿caval perforation: yes.1 o¿clock 4.70 mm grade 2.1:30 o¿clock 7.20 mm grade 2.4 o¿clock 5.60 mm grade 3 abutting the l2 prevertebral soft tissue.8 o¿clock 6.60 mm grade 2.10 o¿clock 7.60 mm grade 2.Tilt: yes.Posterior tilt measuring 9.72 degrees.Migration: yes.Apex of the caval filter is at the level of the right renal vein.¿.
