This device is used for treatment not diagnosis.The ventricular assist system is indicated for use as a bridge to cardiac transplantation and destination therapy in patients who are at risk of death from refractory end-stage left ventricular heart failure.The system is designed for in-hospital and out-of-hospital settings.Investigation of this event is pending and a supplemental report will be sent upon its completion.If information is provided in the future, a supplemental report will be issued.
 
Event Description
It was reported that the ventricular assist device (vad) exhibited five suction events in five minutes and the patient was asymptomatic.The vad remains in use.No patient complications have been reported as a result of this event.

It was further reported that a computerized tomography showed the inflow cannula as being off axis and directed towards the posterobasal wall of the left ventricle.

Event Description
It was further reported the patient had the vad inflow cannula repositioned by dividing the pericardium.Transesophageal echocardiogram (tee) showed the inflow cannula in a good position and they increased the speed of the vad with no further suction alarms.
