(b)(6).(b)(4).The returned autotome rx 49 was analyzed, and a visual evaluation noted that the device did not present any visual damage in the tip or the cutting wire.However, the working length was twisted.The reported event was not confirmed.The failure found could had been generated during manipulation of the device or due to the interaction with the endoscope or with other devices.Based on the investigation the most probable cause for working length twisted is adverse event related to procedure since the adverse event occurred during the procedure and the device had no influence on event.Additionally, all compiled information on this investigation determines that the most probable cause for the reported complaint is no problem detected since the device problem (tip cracked) cannot be confirmed.A review of the manufacturing documentation for this device revealed that no anomalies or deviations related to the event occurred during manufacturing.A labeling review was performed and, from the information available, this device was used per the directions for use (dfu) / product label.
 
Event Description
It was reported to boston scientific corporation that an autotome rx 49 was used in the common bile duct (cbd) during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2020.According to the complainant, during the procedure, the physician noted a visible crack at the tip of the device.The procedure was not completed as the patient could not endure the prolonged procedure.There were no patient complications reported as a result of this event.
