It was reported that the patient experienced heart failure symptoms and presented in clinic.Upon investigation, the left ventricular (lv) lead was found dislodged via x-ray.The lv lead had dislodged into the right atrium and no capture on any vector.The patient had heart failure due to the inactive, non-capturing lv lead.The patient was otherwise stable and was not admitted to the hospital.The patient was later brought into the operating room for lead replacement.At the beginning of the case, a venogram was done to see if a lead could be inserted.However, the patient's subclavian vein appeared blocked, and it didn¿t look possible to get the lead in.No procedure was done and the patient will be followed clinically to see if heart failure symptoms reoccur.The patient was stable.
