It was reported that during preparation of convective radiofrequency water vapor thermal therapy, the device was set-up/primed without the scope inside.
Once the physician placed the device in the scope and did the pretreatment vapor cycle, it seemed that there was tissue/paper inside the tip of the device.
Physician took out the scope, got rid of any saline inside device, and re-inserted scope but still seemed like tissue was inside the needle tip.
The material noted inside the needle tip was white, not very large, looked almost like sloughed tissue.
Storz scope was used during the procedure and it was verified to be clean prior to use.
It was further reported that the device verified to be clean before the scope was inserted.
The device was replaced and another device was used to complete the procedure without clinical complications to the patient.
