Event Description
Information was received by a manufacturer representative (rep) regarding a patient with an implantable neurostimulator (ins).It was reported that the rep attempted to interrogate a new ins with the tablet and got the error message "validation error.System detected invalid data on the device." the following codes were displayed: 010085, 040201, 030200, 080800, 0a0200.There were options to continue or restart the application.The rep indicated they tried both prior to calling, but they got the same message when she attempted to establish telemetry.The rep called back and confirmed they were able to recharge the ins normally, but the controller on its own showed "device not ready." it was reiterated that when the rep set up for the case, everything was working fine.The validation error was seen in the or when they were going to check impedances.The rep tried clicking both continue and exit workflow, but the same message was seen.Another tablet was used and the same message was seen.The ins was implanted as it was thought the issue was related to the tablet since everything worked prior to implant.The device was disabled and re-enabled which led to the no device found message.The rep tried interrogating the ins again but saw a system error 0x17acabdc message and forced the app to restart.It was reviewed that since the tablet messages couldn't be bypassed, the ins would need to be replaced and returned for analysis.

H3: analysis of the implantable neurostimulator (serial number (b)(6)) found that there was a software or firmware anomaly with an unknown cause.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms.This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report.If information is provided in the future, a supplemental report will be issued.
