Since the subject device has not been returned to olympus medical systems corp.(omsc) for the evaluation, the exact cause of the reported event could not be conclusively determined at this time.If additional information becomes available, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed that the angulation of the subject device was not meeting the specification such as the angulation became locked and could not disengage during the inspection at (b)(4).There was no patient injury associated with this report.
 
Manufacturer Narrative
This supplemental report is being submitted to report the withdrawal of mfr report #8010047-2020-07115 and correct in the initial report submitted on october 2, 2020.Olympus medical systems corp.(omsc) was informed additional information from olympus australia that the locked angulation and non-disengaging the lock had not been occurred actually.Therefore omsc concluded that the reported event was not reportable event.
