The suspect device was returned to olympus and evaluated. The evaluation confirmed the user¿s report of five-amp fuse blown and unit would not turn on due to a faulty switch regulator. There was minor rust found on the bottom of the chassis. A worn out lock latch mechanism on the video connector was observed. In addition, the image was flickering. The device was serviced and returned to the user facility.

A user facility reported to olympus that the five amp line fuse was blown and the unit would not turn on. There was no patient injury or harm, associated with the problem, reported to olympus.
