Event Description
According to the reporter, the capsule failed to attach.The capsule was placed as per usual protocol.There was no scope in the patient's mouth.They placed tiny amount of lubricant on back of catheter and not on capsule itself.They passed the delivery system through mouth into esophagus until the desired centimeter marking.They turned on the suction and waited 30 seconds.They pushed the plunger down all the way and rotated clockwise 90 degrees.When the delivery system was pulled out, there was no capsule and it had come off the delivery system appropriately.When they put the scope back in to check placement, capsule was not in the esophagus nor the stomach.They found the capsule in hypopharynx above the vocal cords.They removed it using a snare.There no user harm.
