Description of event according to initial reporter: it is alleged that "on or about (b)(6) 2011, [pt] was implanted with a cook celect ivc filter.The cook filter placed in [pt] was stated to be appropriate for use as a permanent filter or a retrievable filter.On (b)(6) 2019, [pt] underwent a computerized tomography scan ("ct scan") of her abdomen.On or about (b)(6) 2019, [pt] was informed that the ct scan showed that multiple struts are perforating through her ivc and into her small bowel." patient outcome: it is alleged that "[pt] faces numerous health risks, including the risks of death.Because the filter cannot be removed, [pt] will require ongoing medical care and monitoring for the rest of her life.".

Patient allegedly received the celect filter via the left femoral vein.Report from ct (computed tomography): "ivc: presence of an ivc filter below the level of the renal veins.Multiple struts of the ivc filter projects outside the boundaries of the ivc.Few struts extend into the adjacent crossing small bowel loops.Evaluation for ivc patency is somewhat limited due to more arterial phase of acquisition.No focal distention or obvious filling defect to suggest ivc thrombosis.".
