Event Description
|
It was reported when opening the box to the stem, it was found to have poked through the bottom of the box.A backup implant was used.There was no patient involvement or delay to procedure.Attempts have been made and additional information on the reported event is unavailable at this time.

This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Evaluation of the returned product/photographs provided confirmed that all sterile barriers are damaged.In addition, the outer carton is damaged from the stem protruding through the carton.Review of the device history records identified no deviations or anomalies during manufacturing related to the reported event.The condition of the device when it left zimmer biomet is conforming to specification.The root cause of the reported event can be attributed to transit damage.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
