Event Description
|
A physician reported on (b)(6) 2020, the certas valve was obstructed.The valve was used during the procedure and the surgeon flushed the valve, but solution could not go through.The surgeon had to scrub off to get another valve to complete the procedure.There was about 10 minutes surgical delay.No patient injury was reported.

Unique device identifier (udi): (b)(4).The valve was not returned for evaluation therefore, an evaluation of the device could not be performed.Lot number information has been provided; therefore, manufacturing records were reviewed and found no anomalies.The cause(s) of the difficulty reported by the customer could not be determined.If additional relevant information becomes available in the future, this complaint will be reopened, and the respective evaluation performed.Trends will be monitored for this and similar issues.At present, we consider this complaint to be closed.

The certas valve was returned for evaluation: review of the history device records for the product code 82-8804 with lot 4610175 conformed to the specifications when released to stock.Failure analysis - the valve was visually inspected, no defects were noted.The position of the cam when valve was received was at setting 3.The valve was hydrated.The valve passed the test for programming, occlusion, leaks, refux, siphon guard and pressure.No root cause could be determined as the technician could not confirm any problem with the valve at the time of investigation.The possible root cause for "surgeon flushed the valve but solution could not go through" could be due to "outgassing as part of steam sterilization process leaves residue that may bind ball and seat".At the time of investigation no issues were noted with the valve.
