The device was returned for analysis. The reported separation was confirmed.Additionally, a tear was noted on the pod.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other similar incidents reported from this lot.The investigation determined that the pod separation was due to inadvertent mishandling.Based on the reported information, it appears that the pod of the delivery catheter was inadvertently handled while attempting to exchange the short barewire (190 cm) to a long barewire (315 cm) causing the pod to separate. The noted tear is likely due to procedural circumstances. There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
