Event Description
It was reported that the patient's ins was at the elective replacement indicator (eri) and had high impedance on the right side.Adjusting the voltage did not induce any clinical change, however the patient did experience transient visual scintillations in all parts of her vision.It was also noted that they felt brief paresthesia in their right hand when the voltage was increase, and there was a subjective neck rotation as the device was turned off or on.There were no other adverse effects.The ins was explanted and replaced and the issue was resolved without sequela on (b)(6) 2020.The etiology was listed as related to the device/therapy and not related to the implant procedure.
 
Event Description
It was reported that no additional troubleshooting was performed and the cause of the issue was not determined.
