A biomedical technician (biomed) at a user facility reported that a fresenius 2008t hemodialysis (hd) machine had a burned power supply and a melted fan cable.
A patient care technician (pct) was disinfecting the machine when bleach spilled causing the fan cable to melt and fuses on the power supply to blow.
The biomed also stated that a puff of smoke was observed upon powering down the machine.
A patient was not connected to the machine at the time of the incident and there was no harm to any patients or individuals because of this malfunction.
The machine has approximately 32,000 hours and the parts are the original fresenius parts on the machine.
The biomed reported that there was no damage observed on any other components, or any other additional issues, associated with the burned power supply and the melted fan cable the biomed replaced the power supply and the fan cable, which resolved the issue.
The unit was returned to service at the user facility without issue and without reoccurrence of the event.
The power supply and the fan cable were discarded by the biomed and are not available to be returned to the manufacturer for physical evaluation.
