Event Description
It was reported that unspecified bd" connecta had 5 air in the line, 4 flow issues, 9 leakage, 89 disconnected stopcock, 27 kinked tubing, and 2 damaged and open package.The following information was provided by the initial reporter: material no: unknown.Batch no: unknown.It was reported via post market survey that the clinician encountered occurrences of infection (blood stream bacteraemia, sepsis etc.) (46), air embolisms (5), no flow or reduced flow of fluids (4), free or unrestricted flow (2), leakage (9), stopcock inadvertently disconnects from the mating component (62), tubing disconnects from stopcock (27), tubing kinked (27), the bd connecta" stopcock is defective or damaged (2), package seal open before use (2), and the stopcock open in the wrong direction (10).
