It is alleged that the patient received a gunther tulip on (b)(6) 2012.It is alleged that the patient was injured without further explanation.Hospital and medical records have been requested, but not yet provided.

Event Description
Patient allegedly received an implant on (b)(6) 2012 via the right internal jugular vein due to deep vein thrombosis (dvt) and pulmonary embolism (pe).Patient is alleging vertebral body perforation.The patient further alleges "numbness of extremities, impaired cognitive function, vision impairment/ loss, and back pain." report from ct (computed tomography): "there is a greenfield type ivc filter in place within the mid ivc.The superior most aspect of the filter is below the right and left renal veins.There is a lumbar spine scoliosis convexity towards the right which deviates the ivc.The ivc filter is normally oriented within the lumen of the ivc.The four most distal anchoring prongs projecting outside the confines of the ivc wall at the 12:00, 3:00, 6:00 and 9:00 positions.The 12:00 anchoring prongs appears to extend up against the wall of the third portion of the duodenum.The three other anchoring prongs projecting into the retroperitoneal fat.".
