Event Description
It is alleged that the patient received a celect inferior vena cava (ivc) filter on (b)(6) 2016, and the patient was injured without further explanation.Hospital and medical records have been requested, but not yet provided.

Event Description
The patient received an implant on (b)(6) 2016 via the right internal jugular vein due to diagnosed with vein thrombosis and pulmonary embolism.The patient alleges tilt and mesenteric perforation.The patient further alleges shortness of breath, chest pain, numbness in extremities, impaired cognitive function, abdominal pain, and internal discomfort.

Event Description
Additional information provides the patient alleges vena cava perforation.The patient further alleges numerous post filter placement deep vein thromboses (dvt), pulmonary embolisms (pe) after filter placement, and a failed filter removal on (b)(6) 2016, as well as lack of energy, limited activity, and poor circulation.
