Event Description
|
Patient allegedly received an implant via the unspecified groin due to venous thrombosis (vt) and pulmonary embolism (pe).Patient is alleging device tilt, device is unable to be retrieved and organ (mesenteric) perforation.Patient notes and further alleges experiencing "shortness of breath, chest pain, numbness in the extremities, impaired cognitive function, vision impairment or loss, back pain, abdominal pain, internal discomfort", per a computed tomography (ct) abdomen without contrast: findings: an interior vena caval filter is seen.No significant tilt is seen on the lateral study with good positioning anterior to posterior.Minimal tilt is noted on the coronal image with the filter 4 mm from the left lateral wall and 15 mm from the right lateral wall.There is no contact of the tip in the wall of the cava.The filter is approximately 23 mm caudal to the bottom of the left renal vein and 14 mm below the right renal vein.There is no extension of the filter into the iliac, renal, hepatic, lumbar, or gonadal veins.There is no evidence of filter strut bending or fracture.No caval stenosis is identified.The filter drops perforates beyond the margin of the cava including a medial perforation seen on sequence 4 and image 78.The tip is 3 mm medial to the caval wall.A similar defect is noted laterally with the tip 4 mm beyond the caval wall.A posterior caval strut perforation is seen with the strut 5 mm posterior to the posterior caval wall on sequence 5 and image 48.More subtle abnormality is noted anteriorly with the caval strut to 2 mm beyond the caval wall on image 49".
