Event Description
It was reported that, during a navio tka procedure, the insertion of an anspach cutter l-5b into the long attachment revealed a resistance that could hardly be overcome and revealed a spiral-shaped material, there was possibly metal debris.The procedure was successfully completed without delay using a back-up device.No patient injury or other complications were reported.
 
Manufacturer Narrative
H6: the navio long attachment, pfsr110006 intended for use in treatment was returned for evaluation.Nothing was identified visually that contributed to the reported problem.A functional evaluation was performed.The reported problem was not confirmed.The bur passed easily through the long attachment lumen.A review of manufacturing records indicate the device met all specifications upon release into distribution.A complaint history review for similar reported/confirmed complaints associated with the part identified prior events.A relationship between the reported event and the device could not be established as there was no damage or evidence of an obstruction found with the device during the visual and functional inspection.Although the reported problem was not confirmed a contributing factor may be disintegration of the internal bearings causing an obstruction in the long attachment.Although no further containment or corrective action is recommended or required at this time, all complaints are monitored and trended through post market surveillance activities.
