Event Description
|
Information was received from a healthcare provider via a company representative regarding a patient receiving dilaudid 5.0mg/ml at ¿0.2499¿ via an implantable pump.It was reported that the patient came in for a pump refill.When the pump was interrogated, it showed reservoir should be 14ml but when the healthcare provider aspirated the did not get anything out (pump refill was under fluoroscopy).They tried a couple of times but were unable to aspirate.The healthcare provider then infused 9ml of 9ml of preservative free saline without any issues and then he aspirated 9ml back of the saline without any issues.He then decided to infused 9ml of preservative free saline and leave pump at minimum rate and have the patient come back in 2 weeks for a follow up and see if we need surgical intervention or if pump is working.There were no issues reported from the patient or any environmental, external or patient factors that may have led or contributed to the issue.The issue was not resolved at the time of the report and it was noted that the healthcare provider would not have any further information regarding the event.

Additional information was received from the healthcare provider via a device manufacturer representative indicated that at a refill they were expecting 4 ml and got back 17; today when the patient came in they were expecting 15.5 ml and got back 19.

Additional information was received from the healthcare provider via a device manufacturer representative indicated that at a refill they were expecting 4 ml and got back 17; today when the patient came in they were expecting 15.5 ml and got back 19.A dye study was performed around that time and the "catheter looked fine." the caller stated that the patient noted that her pain was coming back and that it was not as well controlled as before.

Additional information was received from a healthcare provider via a company representative.The healthcare provider had reported a large volume discrepancy during refill.Factors that may have led or contributed to the issue remained unknown.The pump was explanted and would not be replaced in the future.The patient did not want to proceed with replacement.The pump was to be returned to the manufacturer.The issue was indicated as having been resolved as of 2022-apr-18.The patient was without injury regarding their status as of 2022-apr-18.
