Event Description
It was reported that the patient had their entire cardiac resynchronization therapy defibrillator (crt-d) system explanted due to endocarditis.An infection on a lead tip was also noted.Antibiotics were administered and the system was replaced approximately two months later.No further patient complications have been reported as a result of this event.

Event Description
It was further reported that the patient had developed shaking chills and low back pain.Upon being seen at the hospital, a transesophageal echocardiogram (tee)revealed two large vegetations attached to the leads.The patient had contracted bacteremia endocarditis subsequent to the device infection.During the extraction of the system a right ventricular (rv) lead perforation occurred with tamponade.An emergency sternotomy, evacuation of a clot and cardiopulmonary bypass was performed along with suture repair of the perforation.Two days later an additional operation was performed for a retained lead remnant in the innominate vein; the piece was successfully removed and the patient was discharged.The system was replaced two months later.The patient is a participant in a clinical study.Additional information received noted blood cultures revealed haemophilus.It was noted that the patient was hospitalized and discharged due to septic shock and presumed pneumonia prior to the reported infection.
