Event Description
It was reported that during a meniscal transplant procedure, the metal "handles" at the proximal end of two meniscus mender broke away from shafts and it was very difficult to manipulate the snare.The procedure was completed without significant delay using the same device with a clip to hold the proximal end of the shafts.No patient injury or other complications were reported.
 
Manufacturer Narrative
The reported device, used in treatment, was not returned to the designated complaint unit for independent evaluation, thus visual inspection and functional testing could not be performed.There was a relationship found between the subject device and the reported incident.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specification upon release for distribution.The instructions for use was reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A review of risk management files found that the reported failure was documented appropriately.An analysis of customer provided image found the hub separated from the shaft of the device.The complaint was confirmed and the root cause was associated with device design.Please refer to the instructions for use for recommendations on proper use of the device and potential troubleshooting methods to prevent future reoccurrence of the reported event.A corrective action for this failure mode is in place.If the product associated with this event is returned at a future date, this investigation will be reopened for evaluation.
