It was reported, the olympus videoscope's angulation became locked during reprocessing and could not be disengaged.According to the initial reporter, the device was stiff and would not return to the neutral setting.There was no patient involvement during this event.

The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the initial evaluation, the user report could not be confirmed.The angulation was not stiff, and the device was flexing back into the neutral position without issue.However, the adhesive on the device was cracked and peeling, exposing the threads.If additional information becomes available following device evaluation, a supplemental report will be filed.

Initial reporter occupation - medical equipment coordinator.This supplemental report is being submitted to provide the results of the manufacturer¿s investigation.A review of the device history record (dhr), a review of the instructions for use (ifu), as well as a historical trending analysis were conducted during this investigation.The review of the dhr did not find any abnormalities or anomalies identified during production.The device met all specifications upon release.The ifu contains the following statements: ¿if the movement of the up/down angulation lock and the angulation control lever are not smooth, the bending mechanism may be abnormal.In this case, do not use the endoscope because it may be impossible to straighten the bending section during an examination.¿ though a specific root cause could not be determined, as a result of the investigation and condition of the returned device, it is believed that the angulation became stiff because the angle release lever was locked.Olympus will continue to monitor the field performance of this device.
