Event Description
Information was received from multiple sources (manufacturer representative, healthcare provider) regarding a patient who was implan ted with an implantable neurostimulator (ins) for spinal pain.It was reported that patient had pocket revision (b)(6) 2020.In (b)(6) 2021, they fell on hands and knees and said felt a slight ¿rip¿ at pocket site.States had discomfort with turning in certain positions and around end of march, pain became more pronounced with movement.States within last week has been even more persistent.When asked she said at one time she mentioned it to the receptionist at dr clinic but she herself never mentioned it to dr (b)(6).States she wants to be seen tomorrow and is having difficulty connecting to charge.

Event Description
It was reported that on (b)(6) 2021- impedances wnl and battery reoriented.The reason for revision was due to ins not sitting flat in pocket.Patient returning to neurosurgery (b)(6) 2021 for surgical consult for battery pocket.Patient opted for pocket revision.Unknown if issue is resolved.
