The following information is alleged: the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2004 and the patient's filter fractured and perforated through the ivc.One fractured fragment is abutting the vertebral bodies, and a second fragment is perforating the l3-4 fused vertebral bodies.The patient's filter is also substantially tilted.Hospital and medical records have been requested, but not yet provided.

Patient allegedly received an implant on (b)(6) 2004 via the right jugular vein due to deep vein thrombosis (dvt) in the left femoral vein.Patient is alleging tilt, vena cava/organ perforation, fracture, and organ abutment.The patient further alleges "the filter implanted in my body fractured and perforated through my ivc.One fractured fragment is abutting the vertebral bodies, and a second fragment is perforating my l3-4 fused vertebral bodies.The filter is also substantially tilted," as well as anxiety and worry.
