Event Description
|
A customer reported that during a cataract vitrectomy surgery, an ophthalmic operating console screen turned white and the console stopped working.The console was restarted, but the issue was not resolved.An alternate console was obtained in order to complete the surgery.There was no patient harm.

|
The company service representative examined the system and replicated the reported event.The company service representative found an issue with the memory of the host module.The company service representative performed cleaning and lubrication of the module to resolve the issue.The system was then tested and found it to perform as intended.The system manufacturing device history record (dhr) was reviewed.Based on qa assessment, the product met specifications at the time of release.The root cause of the reported event can be attributed to component wear of the host module.Manufacturer will continue to monitor data for evidence of adverse trending and take further action, as appropriate.The manufacturer internal reference number is: (b)(4).
