Event Description
It was reported that 3 bd syringe 0.3ml 30ga 8mm hub separated from the device.The following information was provided by the initial reporter : the patient reported three syringes that the needle stuck in the protective cover and when the shield is removed the needle is stuck together with the shield making its use impossible.
