The subject device was not returned to omsc for evaluation but was returned to omsi.Omsi checked the subject device and found that the reported phenomenon was caused by the failure of the front panel unit.The exact cause of the reported event could not be conclusively determined at this time.If additional information is received, this report will be supplemented.
 
Event Description
Olympus medical systems corp.(omsc) was informed that during the incoming inspection at olympus medical systems india (omsi), it was found that all indicators of the front panel were continued to light up.The subject device is the loaner asset of omsi.There was no report of patient injury associated with the event.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the subject device evaluation result.The device was inspected at olympus medical systems india (omsi).Olympus medical systems corp.(omsc) reviewed the manufacturing history (dhr) of the subject device and confirmed no irregularity.Based upon the information from omsi, omsc surmised that the reported phenomenon was attributed to the failure of the front panel unit and the printed-circuit board due to repeatedly long-term use because over eleven years had passed from the subject device had been manufactured.
