Event Description
A user facility reported to olympus that, the evis exera ii video system center brightness adjustment does not work.There was no harm, infection or user injury reported due to the event.
 
Manufacturer Narrative
The device was returned for evaluation.The evaluation was unable to confirm the reported event of the auto iris and the auto brightness was not working.The video processor was tested with a light source and video images functioned normally.The auto adjustment was also working okay.The device was inspected and passed olympus functional standards.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time.However, if additional information becomes available this report will be supplemented accordingly.

The supplemental report is submitted to provide the result of the legal manufacturer¿s investigation.The device history record for the subject device was reviewed and it was verified that the device was manufactured in accordance with documented specifications.The legal manufacturer performed an investigation.A conclusive root cause was not identified.The investigation determined that there was no defect in the equipment, but there was a defect with the light source device and the light source cable itself, or the connection of the light source cable was not appropriate.As stated on the ifu (instruction for use) and as a preventive measure, the user manual states: "properly and securely connect all cables.Otherwise, equipment damage or malfunction can result.".
