Initial reporter occupation: non-healthcare professional.Investigation: the reported allegations have been further investigated based on the information provided to date.The following allegations have been investigated: vena cava perforation.Filter interacts with ivc wall, e.G.Penetration/perforation/embedment.This may be either symptomatic or asymptomatic.Potential causes may include improper deployment; and (or) excessive force or manipulations near an in-situ filter (e.G., a surgical or endovascular procedure in the vicinity of a filter).Potential adverse events that may occur include, but are not limited to, the following: trauma to adjacent structures, vascular trauma, vena cava perforation, vena cava penetration.Catalog number and lot number are unknown.The alleged tulip is manufactured and inspected according to specifications.No evidence to suggest that this device was not manufactured according to specifications and nothing indicates that the filter did not perform as intended, e.G.Intended for the prevention of recurrent pulmonary embolism (pe) via placement in the vena cava.Cook will reopen its investigation if further information is received warranting supplementation in accordance with 21 c.F.R.803.56.This report includes information known at this time.A follow-up medwatch report will be submitted if additional relevant information becomes available.
 
Event Description
The following information is alleged: the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2006.Approximately 11 years and 7 months later the patient underwent a computed tomography (ct) scan which revealed that the filter had move since its implantation as several struts of the filter were perforating through the ivc wall.Hospital and medical records have been requested, but not yet provided.

Blank fields on this form indicate the information is unknown, unavailable, or unchanged.Investigation: investigation is reopened due to additional information provided.The reported allegations have been further investigated based on the information provided to date.The following allegations have been investigated: pulmonary embolism (pe), deep vein thrombosis (dvt), stenosis, migration, tilt, leg swelling, back pain, anxiety, depression.New pe as a reported complication, is a known risk in relation to filter implant and is well documented in the clinical literature and in clinical practice guidelines.This is supported by the clinical evidence report established to assess available clinical data to identify and evaluate the clinical safety and performance of the cook vena cava filters.Potential adverse events that may occur include, but are not limited to, the following: pulmonary embolism.Ivc occlusion/ thrombosis, new dvt, ivc stenosis as a reported complication, is a known risk in relation to filter implant and is well documented in the clinical literature and in clinical practice guidelines.This is supported by the clinical evidence report established to assess available clinical data to identify and evaluate the clinical safety and performance of the cook vena cava filters.Potential adverse events that may occur include, but are not limited to, the following: vena cava occlusion or thrombosis, vena cava stenosis, deep vein thrombosis.Filter or filter fragment migration and (or) embolization (e.G., movement to the heart or lungs) has been reported.Filter or filter fragment movement has occurred in both the cranial and caudal direction and may be either symptomatic or asymptomatic.Potential causes may include filter placement in ivcs with diameters smaller or larger than those specified in these instructions for use; improper deployment; deployment into thrombus; dislodgement due to large thrombus burdens; and (or) excessive force or manipulations near an in situ filter (e.G., a surgical or endovascular procedure in the vicinity of a filter).Potential adverse events that may occur include, but are not limited to, the following: filter migration, trauma to adjacent structures.Filter tilt has been reported.Potential causes may include filter placement in ivcs with diameters larger than those specified in these instructions for use; improper deployment; manipulations near an implanted filter (e.G., a surgical or endovascular procedure in the vicinity of a filter); and (or) a failed retrieval attempt.Excessive filter tilt may contribute to difficult or failed retrieval; vena cava wall penetration/perforation; and (or) result in loss of filter efficiency.Potential adverse events that may occur include, but are not limited to, the following: unacceptable filter tilt.Unknown if the reported leg swelling, back pain, anxiety, depression is directly related to the filter and unable to identify a corresponding failure mode at this point in time.Catalog and lot numbers are unknown, however, the alleged tulip is manufactured and inspected according to controls.No evidence to suggest that this device was not manufactured according to specifications and nothing indicates that the filter did not perform as intended, e.G.Intended for the prevention of recurrent pulmonary embolism (pe) via placement in the vena cava.Cook will reopen its investigation if further information is received warranting supplementation in accordance with 21 c.F.R.803.56.This report includes information known at this time.A follow-up report will be submitted should additional relevant information become available.This report is required by the fda under 21 cfr part 803 and is based on unconfirmed information submitted by others.Neither the submission of this report nor any statement contained herein is intended to be an admission that any cook device is defective or malfunctioned, that a death or serious injury occurred, nor that any cook device caused, contributed to, or is likely to cause or contribute to a death or serious injury if a malfunction occurred.
 
Event Description
Patient allegedly received a filter via the right common femoral vein post orthopedic surgery.Patient is alleging device migration and tilt, vena cava perforation and caval thrombosis.Patient notes and further alleges experiencing "swelling in left leg mostly.Off [and] on pain in back lower area", post filter placement pe and dvt, anxiety, depression.Per computed tomography (ct) abdomen and pelvis: "there is an ivc filter in place"."there is fullness of the left common femoral vein consistent with the patient's known deep venous thrombosis"."patient recently diagnosed with pulmonary embolus on (b)(6) 2009".Per a ct abdomen: "ivc filter lies approximately 2.3 cm inferior to the inferior most renal vein.There is approximately 10 degrees leftward tilt of the filter, though in the apex lies near the left ivc wall.No stress fracture is definitely identified.A right anterolateral strut extends into the fat around the ivc.Adjacent organs are unremarkable.The ivc inferior to and at the filter is flattened, suggesting narrowing and/or thrombosis.This may be the cause of the collateral vessels seen in the subcutaneous tissues".

Per ct report, "filter position: below above the renal veins." "filter tilt: yes." "filter penetration: yes." "impressions: the filter is tilted to the left with its proximal cone against the ivc wall.Axial image 64.The anterior right strut penetrates 26 mm through the ivc wall.Sagittal image 63.The anterior left strut penetrates 27 mm through the ivc wall.It penetrates into the left common iliac artery.Coronal image 125.Axial image 82.The posterior right strut penetrates 23 mm through the ivc wall.Coronal image 67.The posterior left strut penetrates 25 mm through the ivc wall.It contacts a vertebra where there are chronic reactive changes.Coronal image 66.Axial image 81.".
