The following information is alleged: the patient received a celect inferior vena cava (ivc) filter on (b)(6) 2017.The filter fractured, with a strut missing.Further, there are multiple perforations by the legs of the filter.Hospital and medical records have been requested, but not yet provided.

Patient allegedly received an implant on (b)(6) 2017 via the right internal jugular vein due to pulmonary embolism (pe).Patient is alleging fracture and vena cava perforation.Patient further alleges "[patient] lived with the anxiety of having a filter that could fail further at any time, but that could not be retrieved without a serious surgery.[patient] worried because his filter fractured and perforated outside of [patient] vena cava." per certificate of death, dated 20dec2020: "immediate cause of death: acute respiratory failure, pneumonia, covid 19".
