Event Description
It was reported that the procedure was to treat a mildly tortuous left anterior descending artery (lad) that is 90% stenosed.During deployment of the 2.75x38mm xience xpedition the stent moved in the artery and was hanging in the lumen of the target lesion.A 2.75x38mm xience xpedition stent was implanted over the previously deployed stent.There was no patient sequela and no clinically significant delay.No additional information was provided.
 
Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation was unable to determine a conclusive cause for the reported device malposition, however, factors that may contribute device malposition include, but are not limited to, incorrect device size for lesion, patient anatomical morphology, and patient disease state.A 2.75x38mm xience xpedition stent was implanted over the previously deployed stent, the treatment appears to be related to operational context of the procedure.There is no indication of a product quality issue with respect to manufacture, design or labeling.
