The customer inadvertently pushed debris into the socket when plugging in the scope paddle, causing the device to smoke and arc.This unit is scheduled for return, but has yet to arrive.Should additional information become available prior to the conclusion of the investigation, a supplemental report will be submitted.

Event Description
|
The customer reported that the evis exera ii video system center experienced burning and arcing in the socket after debris was pushed inside with the scope paddle during a diagnostic procedure.According to the initial reporter, the procedure was completed using another video tower, and the patient's outcome was 'ok'.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The device referenced in this report has not been returned to olympus for evaluation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 13 years since the subject device was manufactured.Based on the results of the investigation, it is presumed that the indicated phenomenon was caused by pushing the fragment into the scope connector socket.A definitive root cause could not be identified.Olympus will continue to monitor field performance for this device.
