The following information is alleged: the patient received a gunther tulip inferior vena cava (ivc) filter on (b)(6) 2006, and the filter subsequently perforated through the patient's ivc and tilted significantly within the ivc.Hospital and medical records have been requested, but not yet provided.

Patient allegedly received the filter via the right groin due to upcoming gastric bypass surgery.Patient is alleging device tilt and vena cava perforation.The patient notes and further alleges experiencing anxiety, worry, depression, and bipolar disorder.
