Event Description
It was reported that during prep of an emboshield nav6 embolic protection system (eps), the filter could not be loaded into the delivery catheter (dc) pod.The dc pod was noted to be crushed and separated into two pieces.The device was not used and there was no patient involvement.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.
 
Manufacturer Narrative
The device was returned for analysis.The reported difficult to insert was unable to be confirmed due to the condition of the returned device.The reported material twisted and separation were unable to be confirmed as the delivery catheter was not returned.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history did not indicate a lot specific quality issue.As the delivery catheter was not returned, the investigation determined a conclusive cause for the reported difficulties cannot be determined.The noted damages to the retrieval catheter (kinked) and the noted damages to the bare wire (bent core, stretched coils) likely occurred due to handling or during packing for return analysis.There is no indication of a product quality issue with respect to manufacture, design or labeling.
