Event Description
It was reported that when the 6.0x40mm armada 18 percutaneous transluminal angioplasty (pta) catheter was opened, the plastic wrapping was torn and sterility of the device was unknown.There was no device use or patient involvement and there was no reported clinically significant delay in the procedure.No additional information was provided.
 
Manufacturer Narrative
Visual analysis was performed on the returned device.The reported packaging damage was unable to be confirmed because the packaging was not returned.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history revealed no other incidents and/or complaints reported from this lot.Based on the reported information and without having the packaging returned, a definitive cause for the packaging damage could not be determined.It may be possible that the damage occurred at the account and the product pouch was inadvertently placed back on the shelf; however, this could not be confirmed.There is no indication of a product quality issue with respect to the design, manufacture, or labeling of the device.
