It was reported that during a cryo ablation procedure, there was resistance when introducing the sheath.
The sheath was removed and the access site was pre-dilated.
The sheath was re-inserted into the patient and there was resistance met, due to scar tissue at the septum.
After crossing the septum, a perforation was noted in the left atrium, that escalated into a cardiac tamponade.
The patient was taken to the operating room (or) for surgical repair.
The outcome of this case is unknown.
The patient was recovering as expected.
It was also reported that the transition from the sheath to dilator transition was not as expected.
No further patient complications have been reported as a result of this event.
