It was reported that bd insyte 20ga x 1.16in was damaged.The following information was provided by the initial reporter: " peripheral venous catheter # 22 is requested for channelization, at the time of puncture return is evident, when advancing the catheter it is evidenced that the catheter opens in two, which again implies puncture of the patient.Patient diagnosis: urinary tract infection.Catalog 38831414, lot 1131191.Date of the event: (b)(6) 2021.".
