Event Description
It was reported that during preparation a three way stopcock was connected to the side port of the dilator; however the dilator could not be flushed.The stop cock was exchanged however it was still impossible to flush and de-air the dilator.A syringe was connected directly to the side port of the dilator but it was still impossible to flush.It is suspected there was a blockage in the dilator.There was no clinically significant delay in the procedure and no patient involvement.No additional information was provided on this device issue.

The device was reportedly retained by the facility.Investigation is not yet complete.A follow-up report will be submitted with all additional relevant information.

The device was returned and investigated.The returned device analysis did not confirm the reported difficult to flush issue.The discrepancy between what was reported (difficulty flushing) and what was observed was likely due to a combination of varying amounts of tension felt by the device during prep versus the returned product analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar incidents reported from this lot.Based on investigation, a cause for the reported difficult to flush could not be determined.There is no indication of a product issue with respect to manufacture, design, or labeling.
