Event Description
It was reported that the procedure was to treat a mildly calcified left anterior descending artery.The 3.5x18mm xience skypoint stent delivery system (sds) was advanced and deployed without issue; however, during removal of the sds the tip separated in the guide catheter.The separated portion was removed with the unspecified guide catheter.There were no adverse patient effects and no clinically significant delay.No additional information was provided.
 
Manufacturer Narrative
The device was returned for analysis.The reported material separation was confirmed.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely the device interacted with the guide catheter during removal causing the noted shaft stretching/damage and reported separation.There is no indication of a product quality issue with respect to manufacture, design or labeling.
