Event Description
The customer reported that they have had an employee health case occurred due to the syringe breaking and shooting into someone¿s eye.Additional information was received from the customer and stated that the syringe broke at the luer tip.The incident happened during a different step of mixing.The medication used during the incident was not indicated.The customer further stated that there was no harm occurred to the staff involved and there was no medical intervention or treatment required.
 
Manufacturer Narrative
The complainant indicated that the device will not be returned for evaluation; therefore, a failure analysis is not available, and we are not able to determine the relationship between this device and the cause for this event.  if additional information or the sample is received, the investigation will be reopened and responded to accordingly.

A device history record review could not be performed because a lot number was not received with the complaint.As part of our manufacturing process, all device history records are reviewed and approved by quality, prior to release of product.A physical sample was not received for the investigation.Because a sample was not returned, we were unable to perform a follow up investigation to include functional and visual evaluations to confirm the issue and root cause analysis.At this time, a corrective and preventive action is not deemed necessary.We will keep monitoring the process for any adverse trends that require immediate attention.This complaint will be used for qa tracking and trending purposes.
