Event Description
The customer reported the visera pro xenon light source xenon lamp does not switch on.The event occurred during procedure and there was no patient harm or user injury reported due to the event.
 
Manufacturer Narrative
The device was returned for investigation.Upon evaluation of the device, the reported issue of lamp failure was confirmed due to interlock switch that became dislodged from its position.The lamp was unable to light up and emergency lamp indicator was blinking due to harness that was disconnected.In addition, missing screw on the main board was observed.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.Information obtained on the repair of the device confirmed that the device would not start, and this was due to a faulty interlock switch.This investigation presumes that the interlock switch was attributed to the phenomenon, but the cause of the fault could not be further determined.Olympus will continue to monitor field performance for this device.
