|
It was reported that blood was backing up in the peripherally inserted central catheter (picc) line while being used with a clearlink, non-dehp solution set.After troubleshooting, a crack was observed in the tubing at the filter.Upon further inspection, the leak was observed due to an incomplete connection between the distal end of the tubing before the 0.2 micron filter; the filter appeared intact.This issue resulted in clotting of the picc line while the solution was not infusing.There was no report of patient injury or medical intervention associated with this event.No additional information is available.

H10: the device was received for evaluation.Visual inspection was performed using the naked eye which did not identify any abnormalities that could have contributed to the reported condition.A functional testing was performed including clear passage and pressure testing; and no leaks were observed.Additional priming was performed which revealed a leak in the vent of filter.The reported condition was verified.The cause of the condition was due to supplier related issue.Should additional relevant information become available, a supplemental report will be submitted.
