Event Description
It was reported that blood was backing up in the peripherally inserted central catheter (picc) line while being used with a clearlink, non-dehp solution set. After troubleshooting, a crack was observed in the tubing at the filter.Upon further inspection, the leak was observed due to an incomplete connection between the distal end of the tubing before the 0.2 micron filter; the filter appeared intact. This issue resulted in clotting of the picc line while the solution was not infusing. There was no report of patient injury or medical intervention associated with this event. No additional information is available.
