The device was returned for investigation.Upon evaluation of the device, preliminary lamp not turned on due to lamp and transformer failure were noted.In addition, brightness problem, intermittent failure to turn off the switch, and lamp not turning on automatically were observed.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.
 
Event Description
A user facility reported that the visera xenon light source lamp does not turn on automatically.The event occurred during preparation for use.During a standard service inspection of the customer returned device, main lamp failure to turn on was noted.This report is to capture the reportable malfunction found at inspection.There was no patient involvement, no harm or user injury reported due to the event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.The dhr was unable to be reviewed for this device since the device is over 15 years old.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.The case is not due to design.Based on the results of the investigation, it is likely that the phenomena occurred due to the faulty transformer and the faulty lamp.A definitive root cause cannot be identified.Olympus will continue to monitor the field performance of this device.
